OBTAIN
A SICKNESS CERTIFICATE?
You do not require a doctor's sickness certificate for any illness
lasting seven days or less. Your employer will be able to give you
a self-certification form to fill in.
If you have an illness keeping you off work longer than seven days
you will need to see your doctor for him to issue you a sickness certificate
(this must be a routine and not an urgent appointment).

WITH THE PRACTICE?
If
you are new to the town and need a doctor, please come to the surgery and bring your NHS medical card with you. If you do not have a card the receptionist will give you a form to complete.
